Delegation does not transfer accountability

A home nursing agency performs study procedures. The investigator remains responsible for their conduct within the trial.

That is not a formality. It means their qualification, training and performance are things you have to be able to evidence.

Know who actually attends

Agencies rotate staff. The nurse who attends visit 3 may not be the one who attended visit 1.

For study procedures, the individual performing them generally needs to be identifiable and appropriately trained. "The agency" is not a person.

Study-specific training is not optional

A nurse can be excellent and unfamiliar with this protocol. The assessment scale, the sample handling, the timing requirements and what counts as reportable are all study-specific.

Confirm that training happened and that it is documented, for the individuals who attend.

Give them what they need before the visit

The visit requirements, the timing windows, the sample handling instructions, and the escalation route.

A nurse in a participant's home with an unanswered question has to improvise. That is a failure of preparation, not of the nurse.

Read their reports as data, not paperwork

A home nursing report is source data for the assessments it records. Read it on arrival, and query it while the visit is recent.

Filed unread, it is the decentralized version of an export that was never run.

Check the timing against the window

The agency schedules for the participant's convenience. They do not always know that visit 5 has a three-day window.

Confirm the booking is inside the window before it happens.

Raise performance problems through the contract

Repeated late reports, missed escalations or untrained staff are contractual matters, and they are also protocol compliance matters.

Report them to the sponsor. An agency issue absorbed quietly at site level is invisible to the people who can fix it.
