- 1974: Establishment of the FDA In 1974, the Food and Drug Administration (FDA) was established in the United States. The FDA plays a crucial role in regulating and overseeing clinical trials to ensure the safety and efficacy of new drugs.
- 1981: First Guidance on Good Clinical Practice (GCP) The World Health Organization (WHO) published the first guidance on Good Clinical Practice (GCP) in 1981. This document laid the foundation for ethical and quality standards in clinical research.
- 1986: ICH Harmonization Efforts Begin The International Conference on Harmonisation of Technical Requirements for Registration of Pharmaceuticals for Human Use (ICH) was founded in 1986 to promote global harmonization of regulations and guidelines for pharmaceuticals.
- 1996: Publication of ICH E6 Guideline The ICH E6 Guideline for Good Clinical Practice was published in 1996. This guideline provides a unified standard for the design, conduct, monitoring, auditing, recording, analysis, and reporting of clinical trials.
- 2000: Declaration of Helsinki Revision The Declaration of Helsinki, a set of ethical principles for medical research involving human subjects, was revised in 2000 to incorporate modern standards for protecting research participants.
- 2001: Formation of TransCelerate Biopharma In 2011, TransCelerate Biopharma was formed by a group of biopharmaceutical companies to collaborate on improving the quality, efficiency, and safety of clinical trials.
- 2003: EU Clinical Trials Directive The European Union Clinical Trials Directive (2001/20/EC) was implemented in 2003 to harmonize the regulation of clinical trials across EU member states and ensure the protection of trial participants. It has since been repealed — see the 2022 entry below.
- 2008: FDA Amendments Act The FDA Amendments Act of 2007, which came into effect in 2008, introduced new requirements for clinical trial registration and results reporting on ClinicalTrials.gov.
- 2010: Launch of the Clinical Trials Transformation Initiative (CTTI) The Clinical Trials Transformation Initiative (CTTI) was launched in 2010 to drive improvements in the quality and efficiency of clinical trials through collaboration between stakeholders.
- 2012: EMA Revision of GCP Guideline The European Medicines Agency (EMA) revised its guideline on Good Clinical Practice in 2012 to align with the ICH E6 Guideline and reflect current best practices.
- 2015: Revised Common Rule The U.S. Department of Health and Human Services revised the Common Rule in 2015 to strengthen protections for research participants and modernize regulations governing human subjects research.
- 2016: Launch of the ICH E6 R2 Addendum The International Council for Harmonisation (ICH) released the E6 R2 Addendum to the Guideline for Good Clinical Practice in 2016, introducing new requirements for quality management in clinical trials.
- 2018: GDPR Impact on Clinical Research The General Data Protection Regulation (GDPR) came into effect in 2018, impacting the way clinical research data is handled and requiring greater protection for personal data of trial participants.
- 2020: COVID-19 Pandemic Accelerates Digital Transformation The COVID-19 pandemic in 2020 accelerated the adoption of digital technologies in clinical trials, leading to innovations in remote monitoring, telemedicine, and virtual patient visits.
- 2021: Continued Focus on Diversity and Inclusion In 2021, there was a continued emphasis on promoting diversity and inclusion in clinical research to ensure that trial populations reflect the diversity of patient populations and improve the generalizability of study results.
- 2022: EU Clinical Trials Regulation Applies The EU Clinical Trials Regulation (EU) No 536/2014 came into application on 31 January 2022, repealing the 2001 Clinical Trials Directive (2001/20/EC) and moving trial applications to a single Clinical Trials Information System (CTIS). The transition period for trials still running under the Directive ended on 31 January 2025.
- 2022: Advances in Real-World Evidence (RWE) In 2022, there were significant advances in leveraging real-world evidence (RWE) from sources like electronic health records and wearables to complement traditional clinical trial data and inform regulatory decision-making.
- 2023: Expansion of Decentralized Clinical Trials Decentralized clinical trials, which allow for remote participation and data collection, saw significant expansion in 2023 as sponsors embraced innovative trial designs to increase patient access and retention.
- 2024: Directions in GCP Through 2024, Good Clinical Practice continued to move toward further integration of technology, increased focus on patient-centricity, and ongoing efforts to streamline regulatory requirements and enhance data quality in clinical research.
- 2025: Adoption of ICH E6(R3) The International Council for Harmonisation adopted E6(R3) on 6 January 2025, superseding the 2016 E6(R2) addendum. E6(R3) restructures GCP around 11 overarching principles plus Annex 1 for interventional trials, with materially greater emphasis on quality by design, risk-proportionate approaches, data governance and fitness for purpose. In the EU the principles and Annex 1 took effect on 23 July 2025, and the FDA issued its corresponding guidance in September 2025.
- 2026-2027: ICH E6(R3) Annex 2 Annex 2 — additional considerations for non-traditional interventional trials, such as pragmatic and decentralized designs and those drawing on real-world data sources — was adopted by ICH on 3 June 2026 and comes into effect on 15 January 2027.
