Handover and Continuity
Part of Trial Pharmacy (PHARM) Training: Working With the Study Team.

Why this matters
Pharmacy runs on continuity. Studies last years and staff do not.
Most of what a trial pharmacy knows is held by one or two people, and it leaves with them unless it was written down.
What must survive a handover
Which studies are active and where each file is. Current protocol and manual versions. Open queries and their status. Pending excursion decisions and what is quarantined. Expiry dates coming up. Resupply requests in flight. Who to contact at the sponsor for what.
A one-page status per study, updated weekly, does most of this.
Delegation does not transfer
A new pharmacist needs their own delegation, dated before their first task, and their own training record.
This is the most common gap when cover is arranged at short notice. Arrange the signature before the first day, not during the first week.
Quarantined product is the highest-risk handover item
Product in quarantine looks like product. If the incoming person does not know why it is there, it can return to the shelf.
Label it with the reason and the date, not just the word. "Quarantine, excursion 14 Sep, awaiting sponsor release" survives a handover; a yellow sticker does not.
Write the handover down
A verbal handover in a busy week is forgotten within days.
Write it, date it, keep a copy in the file. If something later goes wrong, the note shows what was known and when.
In practice
You are leaving for two weeks. Before you go, you update the status sheet for each active study, confirm the covering pharmacist's delegation is signed, walk them through quarantined stock and the reason for each item, and leave a written handover in the file.
What this looks like when it goes wrong
A pharmacist leaves without recording that a batch was quarantined pending a stability decision. The label says only "hold".
Three weeks later, during a stock tidy, it is returned to the shelf and dispensed to two participants.
The decision never came back because nobody was waiting for it.
Key takeaways
- Keep a one-page status per study, updated weekly.
- Delegation does not transfer; arrange it before cover starts.
- Label quarantine with the reason and date, not just the word.
- Write handovers down and file them.
