Close-Out and Archiving

Part of Trial Pharmacy (PHARM) Training: Inspection Readiness.

About 2 minutes. Free, in your language, no account needed.

Why this matters

Close-out is where pharmacy accountability is finally proved or finally fails.

Everything deferred during the study arrives at once, and anything that cannot be reconstructed now never will be.

What close-out requires

A final reconciliation that balances. All returns received, counted and recorded. All product destroyed under authorization, or returned, with certificates filed. Temperature records complete. The pharmacy file complete and indexed.

The monitor usually verifies accountability before final destruction is authorized. So destruction is the last step, not a tidy-up before it.

Start early

A study that ran four years cannot be reconciled in an afternoon.

Begin while participants are still enrolled. Confirm each month closes as you go, so close-out confirms rather than discovers.

Archiving

Pharmacy records are kept for the period the protocol and local law require. That is often many years after the study ends.

They must stay readable and findable for that whole period. Thermal paper fades, so temperature printouts are copied or scanned. The copy is certified.

Know who holds them, where, and how they would be retrieved if requested in eight years.

Do not discard anything early

Worksheets, correspondence and superseded versions are part of the record.

A drawer cleared during a move has ended more than one site's ability to answer an inspection question.

In practice

At close-out the balance is short by one unit. You check quarantine, then returns not yet logged, then the destruction certificate. It was destroyed and never entered in the log.

You add it as a late entry with the certificate reference. The balance closes. The record explains itself.

What this looks like when it goes wrong

A site puts pharmacy records in a basement. Nobody checks the retention period or records the location.

Four years later a regulator asks for temperature records. The boxes cannot be found. The site cannot show storage compliance for a completed study.
